Brookcrest, A Christian Life and Rehabilitation Center, provides a wide range of extensive medical and rehabilitative services to meet the needs of residents transitioning from hospital to home. The comprehensive treatment plan offers physical, occupational, speech and recreational therapies to help residents regain their mobility and prior level of independence.!e innovative health care services for seniors provide 24 hour care with the rehabilitation goal of returning home or to the least restrictive level of care. Brookcrest provides skilled nursing care for residents in both private and shared environments.

Registered and licensed practical nurses, certified nurse assistants (CNA), recreational therapists, physical, occupational, and speech therapists, dieticians and medical social workers comprise the core of the Brookcrest team. In addition, each member of the Brookcrest interdisciplinary team completes comprehensive training in Alzheimer’s/dementia and provision of quality care. Brookcrest is fully licensed by the State of Michigan, Medicare and Medicaid certified and an approved Blue Cross of Michigan provider. Brookcrest also accepts private pay as well as most private insurance plans.

Established in 1997, Meals at Home is a volunteer service delivering meals 365 days a year to the home bound. All homemade meals are prepared by the staff at the Sunset Manor & Villages kitchens.

Meals at Home service also offers “Grocery Express”, a next day grocery delivery service. All the staples and small items are delivered from the General Store at Sunset Manor & Villages and brought to your doorstep with a smile.

Meals are only $5.00 a day and include:

Meat

Potato

Salad

Vegetable

Dessert

Bread

Milk

The Meals at Home service areas include: Grandville, Jenison and Hudsonville.
